Chattahoochee Plastic Surgery asks that you fill out forms prior to your consultation. We want to understand your entire history prior to speaking with you or performing any surgery procedures on you. We do this to ensure that you are a good candidate for cosmetic surgery and so that we can learn a little bit more about you to ensure we give you the best results possible.
If your insurance will cover some or all of your surgery, we also ask that you bring your insurance information to your consultation. Prior to your consultation, we ask you to fill out all of our forms as best as you can. If you ever have any questions regarding the questions or your answers, you are always welcome to ask.
